(ii) contacting me in connection with the above event and other future activities/events organized or handled by Jesus Youth Singapore

(b) All photos, videos or audio recordings obtained and/or derived from the above event which may contain my image/voice or any other proprietary rights may be used freely for publishing and archival purposes, on Jesus Youth Singapore and affiliated websites, publications, social media and for future publicity purposes unless otherwise stated in writing.
(c) If you are providing someone else’s personal data or submitting this form on behalf of another individual, you hereby declare that you have obtained the consent of the named individual(s) in this Form, for the collection, usage, processing and disclosure of his/her ‘Personal Data’ by you to Jesus Youth Singapore
