By voluntarily providing my personal data in this form, I consent to Jesus Youth Singapore collecting, using, disclosing and processing my personal data for the following purposes:
(a) My ‘Personal Data’ will be collected, used, disclosed and processed for the purposes of:
- processing my registration and participation in the above event or programme;
- administering, organising, facilitating, communicating with me regarding, and evaluating the above event or programme;
- informing me of future events, programmes, formation activities, ministries and initiatives organised by or on behalf of Jesus Youth Singapore; and
- where reasonably necessary, sharing my personal data with Jesus Youth International, its affiliated ministries and publications solely for purposes related to the administration, coordination, pastoral care, formation, communications and participation in Jesus Youth programmes and initiatives.
(b) Photographs, video recordings and audio recordings taken during the above event or programme, which may contain my image, voice or likeness, may be collected, used, reproduced, published, distributed and archived by Jesus Youth Singapore, Jesus Youth International and their affiliated ministries for pastoral, formation, evangelisation, communication, promotional, historical and archival purposes, including publication in printed materials, websites, newsletters, reports, presentations and official social media platforms, unless I have informed Jesus Youth Singapore in writing prior to the event that I do not consent to such use.
(c) Photographs, video recordings and audio recordings captured during Jesus Youth Singapore events or activities may also be shared with The Roman Catholic Archdiocese of Singapore, including its offices, ministries and agencies, for archival, pastoral, communications, evangelisation and publicity purposes. By participating in such events or activities, I consent to The Roman Catholic Archdiocese of Singapore collecting, using, reproducing, publishing and distributing such photographs, video recordings and audio recordings in its print publications, digital platforms, websites and official social media channels, without payment or further notice, in accordance with its applicable data protection and privacy policies.
(d) If I am providing another individual’s personal data or submitting this form on behalf of another person, I confirm that I have obtained that individual’s consent for Jesus Youth Singapore to collect, use, disclose and process his or her personal data for the purposes stated above.
